Request for a late drop of individual CLAS course(s)

This form should be used by any undergraduate student to request collegiate authorization to drop College of Liberal Arts and Sciences (CLAS) courses from the current semester/session after the published deadline. See the policy information for more details.

Furthering Language Incentive Program (FLIP) credit request

This form should be used by any undergraduate student to request FLIP credit for a course taken in the current or a previous semester. See the policy information for more details.

Independent study contract

This form should be used by any undergraduate student registering for an independent study course in a College of Liberal Arts and Sciences (CLAS) department. CLAS students may earn up to nine semester hours (s.h.) toward the 120 s.h. required for degree using independent study. Academic record requests

Second-grade-only option (SGO) request

This form should be used by any College of Liberal Arts and Sciences (CLAS) student to request a second-grade-only option (SGO) for a course they are currently or previously enrolled in for the second time and would like the first grade to no longer be included in GPA calculations. Request for late/retroactive withdrawal of entire registration

This form should be used by any College of Liberal Arts and Sciences (CLAS) student to request collegiate authorization to withdraw all courses from the current or previous semester/session after the published deadline. This form should be used only by undergraduate students enrolled in CLAS during the semester in question. Restart petition

This form should be used by current or former College of Liberal Arts and Sciences (CLAS) students who have returned to the University of Iowa to complete their first undergraduate degree after an enrollment gap of at least four years, and who want to have one for more CLAS semesters/sessions removed from GPA calculations. Major and program of study requests

CLAS major program of study change request

This form should be used by any currently enrolled College of Liberal Arts and Sciences (CLAS) student, or their academic advisor, who wants to add, drop, or update an existing CLAS major (or "pre-" or "interest" advising designation), other than Computer Science. Computer science—add or drop major

This form should be used by any currently enrolled College of Liberal Arts and Sciences (CLAS) student who wants to add or drop a major (bachelor of arts or bachelor of science) in computer science.

Request to extend lacks 60 sh major declaration hold

This form should be used by any currently enrolled College of Liberal Arts and Sciences (CLAS) student, or their academic advisor, who has an active "lacks 60 sh major declaration" registration hold and wants to request that the hold be extended to a future semester. Communication studies—update to current major or minor requirements

This form should be used by College of Liberal Arts and Sciences (CLAS) students who declared a major or minor in communication studies prior to fall 2026 and want their degree audit updated to the current major or minor requirements. Change of college application

To be used by students wanting to change to a different undergraduate college, or add a second major in another undergraduate college (add an undergraduate "combined degree program").

Discontinuation of undergraduate combined degree program

To be used by students currently in an undergraduate combined degree program to exit a college of enrollment and remove all majors from that college.
